Absolute

  • Combination with drugs highly dependent on CYP3A whose increase may cause serious events, and with inducers that eliminate efficacy, including rifampin, St John’s wort, alfuzosin, amiodarone, quinidine, pimozide, cisapride, lurasidone, oral midazolam, ergots, triazolam, lovastatin, simvastatin, and sildenafil for pulmonary arterial hypertension.

Hyperglycemia, ketoacidosis, bleeding in hemophilia, and severe skin reactions

Pregnancy and lactation

Consider an alternative antiretroviral during pregnancy because of hepatic risk and lower exposure in the second and third trimesters. If nelfinavir is used, monitor closely. Avoid breastfeeding in treated people with HIV.
