Approved indications
- Treatment of hypertension, to lower blood pressure. Lowering blood pressure reduces the risk of fatal and nonfatal cardiovascular events, primarily strokes and myocardial infarctions.
- Treatment of diabetic nephropathy in hypertensive patients with type 2 diabetes, an elevated serum creatinine, and proteinuria.
Contraindications
Absolute
- Hypersensitivity to irbesartan or any of the excipients.
- Second and third trimesters of pregnancy.
- Concomitant use with aliskiren in patients with diabetes.
- Severe renal impairment (CrCl < 30 ml/min).
Clinical warnings
- Boxed warning · FETAL TOXICITY: When pregnancy is detected, discontinue irbesartan as soon as possible. Drugs that act directly on the renin-angiotensin system can cause injury and death to the developing fetus. — FDA
Drug interactions

Mechanism: Dual blockade of the renin-angiotensin system (RAS), increasing the risk of hypotension, hyperkalemia, and renal impairment.
Recommendation: Contraindicated in patients with diabetes. Avoid in patients with renal impairment (GFR < 60 mL/min).

Recommendation: Combination is not recommended. If necessary, monitor lithium levels closely.

Mechanism: May attenuate the antihypertensive effect and increase the risk of renal function deterioration, especially in elderly or volume-depleted patients.

Contraindicated in the second and third trimesters of pregnancy due to fetal toxicity (renal dysfunction, oligohydramnios, death). Discontinue as soon as pregnancy is detected.

Metaanálisis en red de 193 estudios que compara 6 ARA-II. Olmesartán y telmisartán mostraron el mejor equilibrio entre eficacia antihipertensiva y eventos adversos. Valsartán y losartán fueron menos efectivos. Irbesartán se posicionó en un punto intermedio en eficacia y seguridad.
Resultados a 2 años del ensayo fase 3 PROTECT. Sparsentan, un antagonista dual del receptor de endotelina/angiotensina, fue comparado con irbesartán en nefropatía por IgA. Aunque sparsentan redujo más la proteinuria, irbesartán es el comparador activo estándar, validando su rol en la nefroprotección.
Revisión sistemática y metaanálisis en red sobre combinaciones a dosis fija. La combinación irbesartán/amlodipino se clasificó como la primera en reducción de presión arterial sistólica, destacando la eficacia de las combinaciones basadas en ARA-II/Calcioantagonista.
Ensayo fase 3 comparando sparsentan con irbesartán en glomeruloesclerosis focal y segmentaria (GEFS). Sparsentan logró una mayor tasa de remisión parcial de la proteinuria a las 36 semanas, pero no hubo diferencias significativas en la pendiente de la TFG a las 108 semanas. Irbesartán sirvió como un robusto control activo.
Estudio de fase 1 de Zilebesiran, una terapia de ARN de interferencia. Un brazo del estudio evaluó la coadministración con irbesartán, mostrando un efecto antihipertensivo aumentado. Esto posiciona a irbesartán como una terapia de base para futuras combinaciones innovadoras.
