Contraindications

Absolute

  • Hypersensitivity to metronidazole or nitroimidazoles; disulfiram use within the previous 2 weeks. For trichomoniasis, first trimester of pregnancy under the FDA label.
  • FDA profile: Cockayne syndrome, because of the risk of irreversible and fatal acute liver failure. The CIMA profile says not to use unless no alternative exists and benefit outweighs risk.

Seizures, encephalopathy, peripheral neuropathy, SCAR, and severe hepatotoxicity

Pregnancy and lactation

The FDA first-trimester contraindication is limited to trichomoniasis; for other infections, assess benefit and risk. Under the FDA profile, pump and discard milk during treatment and for 48 hours after the last dose; the CIMA profile recommends avoiding breastfeeding.
